· Please send check made out to Gulf Shore Church to 26468 Doverstone St., Bonita Springs, FL 34135 Care Of Carol Blalock.
· Please include the guests name, parents name, phone number, address, and email address with payment.
· **If parent would like to volunteer to help please contact Tara Hahn 239-281-9095 for information.
· Once payment has been received ticket will be emailed to you at provided email address.
· If you would like to pay in person you may do so M - F 9am-5pm at Action One Computers, 24951 Old 41 Suite 5, Bonita Springs, FL, 34135 (Corner of Old 41 and Strike Ln).
